
We are thrilled to have our Director of Marketing, Laura Huisking, present “Caregiving is a Team Effort :The Role of a Care Manager” on March 10th at 1:00pm as a part of March’s The Dementia & Parkinson’s Caregiver Education series. This three part series supports Dementia and Parkinson’s care partners with practical tips and expert guidance, allowing caregivers to connect and learn about resources online.
This educational series was created by Marlene S Ceragno, MA, CPG, CDP, who holds a Master’s in Gerontology from the University of Massachusetts. She is a Dementia Care Practitioner and works as the Director of Caregiver Services and Outreach in the Senior Department at the JCC on the Palisades. Caregiving is a team effort! The caregiver journey can be complicated to navigate so why not bring some help along with you? We prepare so often for other parts of our lives- college, weddings, and vacations. Why don’t we treat caregiving the same way? Extra help can often be a godsend and it can make you feel less alone. Gathering resources that add extra layers of support is essential to making the journey a less stressful one.
Hiring a care manager provides an invaluable layer of support for Dementia and Parkinson’s caregivers. Every family is different, and our care managers are able to devote 100% attention to their clients. Our team helps families examine each unique situation and gives expert guidance about developing the best approach. Our care managers determine care needs with a comprehensive assessment. They develop and implement a care plan that is tailored to the family. They monitor medical, social and home care services as well as communicate and coordinate with professionals, other service providers and family members regarding daily living needs, medical follow-up, psychological and social functioning.
They can also help the family determine the right time for a senior living community as well as placement assistance for those communities, handling the transition from start to finish. They counsel on insurance and senior benefits programs as well as referrals for financial and legal services. They assist with medical and other appointments, manage communication with physicians, and referrals for additional services. If the family lives out of state and needs an on-call advocate for medical emergencies, one of our care managers can meet the client at the hospital and advocate for them, ensuring a safer discharge plan.
