Part One: The Care Managers Role in Navigating Senior Care
One of the most common questions families have is also one of the most important:
“When do we actually need a care manager?”
The answer isn’t always obvious. Sometimes the need becomes clear after a hospitalization or a health crisis. Other times, families gradually realize that managing medications, appointments, caregivers, finances, transportation, and communication with healthcare providers has simply become too much.
A care manager can step in before—or during—a crisis to assess what is happening, identify what needs to change, and help create a plan that works for the individual and the family.
