Episode Two: How can care management work together to improve a person’s quality of life?
Aging often creates a complicated web of relationships. A family may be trying to coordinate information from a doctor while communicating with a home care agency, arranging physical therapy, researching assisted living, speaking with an elder law attorney, and trying to understand what happens next after a hospitalization. This is all while dealing with complicated sibling dynamics, their loved one’s resistance to outside help, and feelings of isolation and stress.
And that’s just Tuesday!
One of the most valuable roles of a care manager is coordination and objectivity. Care managers serve as advocates and communicators, helping make sure that important information doesn’t get lost between providers and that everyone involved understands the plan. This can be particularly valuable when family members live out of state, when siblings have different opinions about care, or when an older adult’s needs are changing quickly.
As Laura discussed during the series, care management can help bring clarity to a situation that otherwise feels fragmented and overwhelming. Caregivers feel alone, isolated and often do not want to ask for help. How do we give them a sense of control during an uncontrollable situation?
