Transitions has been developed to enable an individual to make the change from independent to assisted living as smoothly as possible. Our comprehensive approach, designed to address the needs of the resident, the family and the staff, insures that each resident entering an assisted living community has the opportunity for a successful and enriching experience.
Transitions services to residents include:
- Medical, social and psychological assessments
- Liaison among the resident, family and assisted living staff
- Identification of potential areas of difficulty and recommendations for intervention
- Short-term counseling to resolve adjustment issues
- Transitions services to families include:
- Family consultation and mediation
- Referral to and liaison with community services, geriatricians, health care aide and medical supply providers
- Advocacy and evaluation of the appropriateness of placement
- Transitions services to assisted living communities include:
- Evaluation of potential residents
- Independent assessment of residents experiencing adjustment and/or management difficulties
- New resident adjustment and support groups
- In-service training of staff
