We are looking to hire a Senior Geriatric Care Manager / Clinical Supervisor (LSW or LCSW). This is a full time position with competitive pay (Northern NJ area)
Role Description
Generations Counseling & Care Management is seeking a full-time Senior Geriatric Care Manager / Clinical Supervisor (LSW or LCSW) to join our team in the Northern New Jersey/ Bergen County area. While some tasks can be done at home, this is a position that is required to be located in Northern New Jersey as our main office is in Rochelle Park. Any applicants must be prepared to travel to clients and/or our clinical meetings. This leadership role includes staff supervision, care management, and collaboration with elder law attorneys, guardians, and families. It entails conducting care assessments, crafting individualized care plans in collaboration with clients and families, and providing clinical supervision to team members. The candidate will educate and guide clients through elder care solutions, coordinate services, and act as an advocate to ensure client needs are met efficiently and compassionately. Responsibilities
Provide clinical supervision to care management staff
Report clinical and operational issues to the Director
Conduct geriatric psychosocial and care management assessments
Manage a select caseload of older adult clients
Perform care management assessments for elder law attorneys and fiduciaries
Coordinate care with hospitals, physicians, home care, and facilities
Support clients with dementia, chronic illness, and functional decline
Requirements
MSW required
Active NJ LSW or LCSW license
5+ years geriatric or healthcare social work experience
Prior supervision or leadership experience
Strong communication, documentation, and organizational skills
Driver’s license and reliable transportation
Preferred
LCSW strongly preferred
Experience with elder law or private-pay care management
